SNAP-ED Subject Educator

This position will serve as the Supplemental Nutrition Assistance Program Education (SNAP-ED) Subject Educator for Cornell Cooperative Extension Association of Oswego County and will provide input into the planning for, and conduct the implementation of, public education to improve healthy food and lifestyle choices among low-income families and individuals encompassing all age groups throughout Oswego County. This position will be responsible for the delivery of established SNAP-ED programming to community groups and will deliver other established SNAP-ED programming and activities, both on-site and off-site, in the community, and through remote digital education. As part of the Federal SNAP-ED Regional Program this position works closely with Regional Nutrition Resource Educators, Cornell Cooperative Extension of Oswego Human Ecology Team Leader and the Regional Project Manager, to assist in program evaluation, management and marketing. This position also assists the Social Marketing and Environmental Nutritionist in implementing the social marketing and environmental change initiative in collaborations with other nutritionists of the SNAP-Ed program in the North County region.

Family Support Specialist

This position will serve as the Family Support Specialist for Cornell Cooperative Extension Association of Oswego County and will teach existing educational programs and activities for the Healthy Families Program. This position will provide voluntary intensive home visitation services to overburdened families in Oswego County who are pregnant or have a newborn and will initiate and maintain a long-term trusting relationship with families until the infant is school age or enters Head Start. Will promote positive parenting skills, optimal prenatal care, child health, safety and development and will help strengthen family functioning and foster self-sufficiency through an individualized Family Goal Plan. Will provide and conduct screening per program guidelines and will provide services for up to 20 families. This position may provide data entry and management of participant data, requiring confidentiality.

Family Resource Specialist

This position will serve as the Family Resource Specialist for Cornell Cooperative Extension Association of Oswego County and will provide program delivery and administrative support to the Healthy Families Program. This position will receive screening documents and contact families to begin engagement in the Healthy Families Program; will conduct home visits to perform parent survey assessments and provide initial referrals for all prospective families; will provide initial education to families on topics including, but not limited to: prenatal/postnatal care, child safety, available resources, etc. This position will also conduct case conferences with Family Support Specialists and Supervisor to review family strengths, stressors, and needs and will provide outreach to community referral sources including, but not limited to: hospitals, health centers, school, private practices, community-based organizations, churches, local businesses, etc. This position will utilize existing program resources and materials in program work.
